If you have worked 1250 hours & been employed with Indiana Packers Corporation for a year, you may quality for FMLA. You will need to see Melissa Johnson in First Aid to initiate a claim. If you do not qualify for FMLA, you can request a personal medical leave of absence from First Aid. You must have proper documentation for these leaves considered for approval.
I have points that are supposed to fall off in November, will those points fall off in this new policy? If I have a Dr.’s note that states I cannot return to work for 3 days, does this excuse me from points?
