Yes. Team members are able to schedule PTO after they have completed their first 60 days of employment. In this situation, the team member will have already earned 6.92 hours.
Yes, as long you have the total # of hours you are requesting in your UTO and/or PTO bank. Keep in mind, PTO can only be used in 4 or 8 hour increments.
You must have the total # of hours you are requesting in your UTO bank before a Supervisor is allowed to approve your request.
If you do not have a leave approved to cover this time, points will apply.
If you have worked 1250 hours & been employed with Indiana Packers Corporation for a year, you may quality for FMLA. You will need to see Melissa Johnson in First Aid to initiate a claim. If you do not qualify for FMLA, you can request a personal medical leave of absence from First Aid. You must have proper documentation for these leaves considered for approval.
With this new time-off policy the review period has changed from a 365 day review to a 90 day review. Point values have also changed from 30 to 8. With that being said, team members will lose 1 import point (points that you started with on day one of this new policy term) every two weeks. Any occurrence point(s) received will stay true to the new policy terms with a 90 day review period. Please see policy transition table below to view import points.

Yes, you will receive 1 point for the hour not scheduled and you will have 3 hours of UTO deducted from your UTO bank.
No, you will only lose 25 min of UTO. UTO can only be scheduled in 1 hour increments but the actual time missed is what will be pulled from your UTO bank. You will also receive 1/2 point for being under 30 minutes late.
No, UTO & PTO must be requested and approved by the end of your prior shift. If you need to miss any scheduled working hours on the same day you are requesting, you must use your points unless otherwise covered by an excused absence such as Jury Duty, Bereavement, etc.
